第一次看到這個病人是在照會的時候，照會+討論會=一群人塞住醫院的通道(像是thrombus一樣!XD)，這個有點年紀的病人因為左眼疼痛和腫脹而來醫院，不過他厲害的地方是他已經忍耐了一個月，過去病史只知道有高血壓，看起來像原住民，不過不喝酒，也不抽菸和吃檳榔，整個人很精瘦，看起來很健康，血液檢查也只發現ESR有升高，反而CRP還是正常的。
他的左眼並不會特別突出，儘管他的左邊臉頰已經真的瘦到皮包骨，看到他的第一個印象，如果只看左臉，真的會有看到骷髏頭的感覺(幸好她不會發出恐怖的聲音，反而相當害羞靦腆)，他的診斷是Corneal ulcer、left peripheral type focal pathy、Lagophthalmos(兔眼)、Ectropion(眼瞼外翻)。
因為absent response of left ipsilateral R1 & R2 reflexes and contralateral R2 reflex，所以她的左眼無法閉合，距離大約有一公分，而且左臉的肌肉已經大多atrophy了。
眼睛無法閉合的處理是睡前貼3M膠帶，記得要用棉棒或棉花墊在睫毛和膠帶之間，膠帶要橫著由鼻側往耳側的方向貼。
Muscle atrophy的處置目前有電療(電在肌肉的belly[中間])，也建議他自己以及照顧他的人可以多幫她按摩，按摩的方式是順著肌肉的走向，從origin到insertion，一日數次。至於熱療，因為他還在病毒感染的狀況，所以沒有採用。
藥物的話，目前給他Vit. B12、抗病毒藥，口服類固醇只適合在急性期(一個禮拜以內)使用[體重 x 1mg/day]。
Facial nerve: 第七對的腦神經；起源於橋腦pons(顏面神經核所在地)，會先與來自橋腦和下大腦角之間的nevus intermedius(中間神經)匯合，走在第六、第九對腦神經之間，一起走到IAC(Internal acoustic canal，內聽道)，走出內聽道(全長8mm)後，再走到Geniculate ganglia，接著繼續走到pyramidal eminence(此時還是在temperal bone裡面)，再來會往下走到stylomastoid foramen，並在離開stylomastoid foramen之後分成許多分支。

- Labyrinthine segment[IAC-geniculate ganglion]: nevus intermedius joins facial nerve at the ganglion；this ganglion fibers for taste synapse；此段最窄也最易被血管擠壓到。
- Tympanic segment
- Mastoid segment: three branches(nerve to stapedius、chorda tympani、nerve from the auricular branch of the vagus nerve[pain fiber to 外耳道的後部])
- Extratemporal segment: 9個分支(外耳道和鼓膜的感覺支、nerve to the posterior belly of the digastric muscle and the stylohyoid muscle、5 branches divided after entering the parotid gland[temporal、zygomatic、buccal、mandibular、cervical])。
Facial nerve 的 blood supply: Labyrinthine artery、superficial petrosal artery、stylomastoid artery。
當我們排除腦外傷、腦腫瘤、感染的已知原因引起的facial palsy之後，就可以把那種不曉得原因的facial palsy稱作Bell's palsy。
可能造成infection的有: leprosy[especially with paralysis of upper face]、chronic otomastoiditis、Lyme disease、HIV infection related facial nerve palsy、Herpes zoster(Ramsay Hunt syndrome)。
最後，很謝謝這位病人親身的教導，因為她忍耐了一個月最後被轉來我們大醫院，我才有機會真正看到這種疾病的狀況，實在由衷的感謝他! 身體的病痛如果超過3-5天還一直持續真的是要及早就醫才好喔!!!
